GP Referral
Aim of the scheme:
To improve and maintain the health and mental well being of inactive adults who are at risk of developing chronic disease.
What is Exercise on Referral?
The Exercise on Referral Scheme provides sedentary individuals and individuals with low risk medical conditions with a positive introduction to being physically active. The introductory exercise programme should enable patients to maintain a physically active lifestyle, without dependence on the exercise professional in the longer term.
Who qualifies for exercise on referral?
The scheme is for adults aged 18 years and over who have existing low risk medical conditions and are considered to be inactive (not currently participating in at least 30 minutes of moderate activity on 3 or more days of the week). Patients must be local residents to access the scheme.
How does the scheme work?
An assessment is made by a healthcare professional such as a doctor or nurse - they provide you with the referral forms and help you fill them out. You bring these referral forms to us at Oxley sports centre where a consultation  takes place and a plan of action is made. A qualified instructor then guides you through a 12 week program offering motivation and regular reviews. Swimming and fitness classes may also be advised during your program here at Oxley. After completing the initial program further discounted membership will be offered to you.

To book your initial consultation and to find out more information please contact Sian Kirk Email: s.kirk@sherborne.com or phone 01935 818270

Induction Process
Initial consultation (45min-1hour) - A detailed health questionnaire will be filled in with an instructor to help us understand you current state of fitness and set your personal goals. You will also be given a basic introduction to the cardiovascular equipment.
Personal Program (45 min) - Your instructor will confirm your understanding of the cardiovascular machines, introduce you to some resistance exercises and produce your program card. You are now free to use the fitness suite
FREE 1 to 1 session 1 (30 min) - A motivational session where you can recap on/progress your exercise regime.
FREE 1 to 1 session 2 (30 min) Your second motivational session will offer alternative or more advanced techniques to progress your knowledge and understanding of fitness and health.
Follow up reviews (45 min)  - These are also free and  available as frequently as you require them – we advise one a month once you are an established user. During these sessions we reflect back to your original goals and adapt your regime to keep you on track.

Health MOT and Body stat Machine

Health M.O.T (30 min) - Your initial health MOT is free and includes weight, Blood pressure, measurements and a composition analysis (body stat) reading.
